Although not dated, it fits my requirement of "could be" 1995. Super multi struck dime planchets (2) bonded together through the pressure of the strikes. The reverse is even more exciting than the obverse!

A major die break resulting in about 35% of the obverse die missing when the coin was struck, so no detail in that area could be struck up. The reverse shows where there was not enough pressure to strike up the reverse details either.
